In 2018, Alf tells Roo that Martha did not drown in 1985, as Roo had been led to believe, and is still alive and out there somewhere. Martha comes to Summer Bay House and meets Ryder Jackson, who asks if she wants to rent a caravan from Alf. It is revealed that Martha knew that Alf’s sister, Morag Bellingham was Bobby’s mother and subsequently had her adopted out to Al (George Leppard; Terence Donovan) and Doris Simpson. Martha Stewart is the first wife of Alf Stewart and the mother of Roo Stewart. Martha left Alf and made Alf tell Roo that she has drowned and died.
Her condition later took a turn for the worse and she suffered a seizure. In order to treat her, the doctors needed to know about her trip and her foster-daughter Maddy Osborne found her passport, which revealed that she had been to Peru. With the information, the doctors were able to treat Roo and save her life. She leaves, but later returns and comes face-to-face with Alf and then Roo. Martha explains that a friend informed her Roo had come looking for her.

Dr Stevens notices that Justin's blood pressure is fluctuating, and then dropping when Christian realises removing the tumour is going to be more complicated than he first thought. On 30 November 2022, Georgie Kearney of 7Entertainment reported that Fox-Reo was set to leave the serial and that he had already filmed his final scenes.
Clarke told David Brown of TV Week that everyone thinks she is making it up when she declares that she is in love, until her boyfriend arrives in the Bay. Clarke added "It's a very good storyline. I'll be able to tackle another teenage problem." She struggled to connect with Roo and could not face up to her condition, so she left. After seeking help and treatment, the guilt and fear stopped her from coming back, but she knew Roo would have a good life. Roo tells Martha that she gave Alf a hard time and had a daughter when she was sixteen, who she named after Martha. Kieran is Martha Stewart's estranged and "deeply troubled" son.

But then they are left reeling from some SURPRISE and unexpected news... Ziggy and Dean are delighted when they see their baby on the monitor for the first time. Ziggy cancelled their original hospital appointment after she was struck by the fear that the ultrasound will lead to the discovery that something is wrong with their unborn baby... Thankfully, good news comes later on from the transplant coordinator, who reveals that Roo is out of surgery and doing well, after which Alf reunites with her. Roo and Martha are later wheeled off to surgery after an understandably nervous Alf bids them goodbye.

The autopsy finds that Christian was not at fault for the patient's death and his suspension is lifted, as the incident happened off site. Lewis tells Jasmine that he is trying to understand what else he has to do to make Christian pay. Christian later asks if they can put their differences aside and concentrate on their jobs, but Lewis refuses. Marco comes to Summer Bay to visit his estranged brother Ben Astoni on his birthday. Despite Ben's anger towards him, Marco assures Ben that he wants to reconcile, as he has missed him. They go out drinking and Marco tells Ben about an investment opportunity.

The casting and character details were confirmed by Women's Day on 17 August 2020. Owen is the estranged twin brother of Evan Slater, also played by Daddo. He said he was "thrilled" when they pitched the character of Owen to him. The character and Jevic's casting details were reported on in the 22 June 2020 issue of TV Soap. Jevic, who was a fan of Home and Away when she was younger, began filming in January and said the cast and crew had been "welcoming and warm" towards her.
It emerges that she called Owen by his brother's name while they were in bed and then left in the night. When Owen eventually turns up, Roo tries to avoid him and he realises that her family dislike him. Roo admits that she did not say anything and they have come up with their own conclusions. Roo struggles to talk to Owen, leaving him frustrated and he storms off.

Jasmine and Tori find them and Tori works to save Christian, while Lewis leaves the hospital. He later shows up in Jasmine's bedroom and tries to explain that Christian was a danger to other patients. Jasmine denounces Lewis for his actions and declares that Anna would not think highly of what Lewis has done if she were still alive. Jasmine then reveals that Lewis's attempt to kill Christian has failed, as Christian is still alive and is recovering, causing Lewis to break down emotionally and beg Jasmine to make it all stop.

Roo's personality gained an unwelcome reception amongst viewers. Clarke said that "everyone" hated Roo and revealed that people would "boo" at her while attending promotional appearances. On one occasion Clarke had to leave a shopping centre because females were shouting "slut" at her. Clarke said that Roo is "a cow" but defended her because Roo ultimately ends up suffering.
Roo starts receiving texts and calls from her boyfriend, Tim, and her bank manager. Colleen Smart is also shocked to see Roo and starts mentioning all the bad things she did in the past, until Alf tells her to stop. Robert Robertson questions Roo about Martha and Hugo Austin for the Federal Police and she states that she has not been in contact with them. Roo later tries to persuade Hugo's mother, Gina , to give them the money Hugo left behind, but Gina is reluctant.

Martha and Alf continues to hang out and talk and when Martha was about to leave the Bay for good, Alf stops her, saying that if she leaves, she'll have another thing coming. Alf and Martha reconcile their relationship and went away for a couple of days. In 2020, Alf and Martha remarried and Martha moves back in Summer Bay with her husband and daughter. The ambulance officer brings Robbo to Northern Districts Hospital following a car crash. Alex Neilson tells her that the ED is closed, but the officer says they would have lost him if they took him elsewhere. Weeks later, the ambulance officer brings Colby Thorne in after he is stabbed in the leg.
